Error 4 - Test Strip Unusable The test strip is damaged or faulty Power off, remove the strip, and reinsert it. If the error repeats, discard and use a new strip
Error 000 - Time Exceeded No blood applied within 180 seconds of prompt Power off, remove the strip, and repeat the test using a new fingerstick
Error 5 - Blood Application Blood applied incorrectly or after timeout Apply blood within 15 seconds; repeat with new stick
Error 6 - Test Strip Interference Strip touched/removed during test Repeat test; avoid touching strip during test
Error QC - Quality Control Failure Internal QC check failed Repeat test with new strip and fresh fingerstick
Error 7 - Measurement Error Bad sample or very long clotting time (>8 INR) Repeat test; confirm by lab if persists
Error 8, Error 9 - Internal Errors Device diagnostic failure or heater contact issue Restart (remove batteries); contact Roche if persists
CoaguChek Pro II
I-001 Battery Low Attach the external power supply or place the meter in its docking station.
E-101 Patient ID Required Enter a valid patient ID and repeat the operation.
E-402 Test Strip Error Check your procedure and repeat the test using a new strip if needed. Power off, remove the strip, and retest with a fresh sample from a new finger. If it recurs, contact Roche Diagnostics.
E-403 Dosing Error Ensure enough blood is applied. Power off, remove the strip, and repeat the test with a new strip and new puncture. If repeated errors occur, the meter may be contaminated — clean or contact Roche.
E-406 Sample Error Unsuitable sample composition or interference. Check procedure and patient medication; repeat test with new strip and fresh sample. If it persists, verify result using another method.
  No Message / Unusual Behavior Wait 10 s and retry power-on. Check battery pack or connect power adapter. If still unresponsive, contact Roche.
  Automatic Shutdown Press the power button to reactivate. Default timeout = 5 min ( configurable by system admin).



Best Practices & Preventive Tips

 
  • Operate and store the meter between 15°C–32°C and <85% humidity (no condensation).
  • Always use CoaguChek test strips specific to the device model and the matching code chip for that strip box.
  • Apply blood within 15 seconds of fingerstick and complete dosing within the meter’s 180-second window.
  • Keep the test strip guide and meter clean; use only recommended disinfectants (70% isopropyl alcohol) and allow drying for at least 10 minutes before testing.
  • Replace batteries promptly and verify meter date/time after battery changes (if batteries were removed >1 minute).
